Spectrum Healthcare Solutions is a network of providers who are committed to improving transitional care and shrinking gaps in care. Although Oklahoma is currently far behind national trends for 30-day readmission rates, we at Spectrum are dedicated to changing this trend.
Because our providers work daily in acute-care hospital settings, they are uniquely qualified to bridge the gap between acute and post-acute care settings, which enables a smooth transition of care and limits negative outcomes during these vulnerable transitions.
Our Goal
Our goal is to ensure the rate of return to acute care (RTA) is less than 15%, which is well below the national average of 22.9%, as well as the Oklahoma's average of 25.2%. By implementing our data-driven approach, the facilities we serve boast an average RTA rate of 11%.
